Writing Clearly but With Confusion

I read medical student patient write-ups. I wonder if students actually understand what they write. They write things like “Patient was alert but unresponsive” or “The patient is tearful and crying constantly. She also appears to be depressed.” Or “no prior history of suicides.” I write: “I understand everything you don’t mean, or maybe not.”
